PUPPY ADOPTION CONTRACT

Adopted Dog has no known behavioral or medical issues. Born with no issues. At this time he/she still needs his/her final DHPP, Bordetella, Spay/ Neuter, Rabies, and Microchip. His/Her Rabies will need be completed between 16-25 weeks old. His/Her Spay/ Neuter needs to be scheduled/completed at 6 months old. He/She is a young puppy and needs to be enrolled in training & socialization to ensure he/she grows into a successfully balanced dog.

I will keep the adopted dog in my own home/yard SOLELY until his/her 3rd set of vaccinations is complete other than taking to/from the vet in which case he/she will be transported via crate and not be placed on the ground to protect from contracting deadly viruses. I will take the adopted dog to a licensed veterinarian within 14 days from the end of the foster period or by the date 3rd set of vaccinations are due, whichever is applicable, and provide proof to RGR within one week of vet visit. I will take the adopted dog to a licensed veterinarian when shots are due but in no event later than one year from the last vet visit per provided vet documents. I will provide all required and/or needed veterinary care, including: rabies shots as required every one or three years; yearly booster shots for DHLPPC; yearly fecal checks for internal parasites (worms); ongoing flea/tick/heartworm preventative, and prompt treatment by a licensed veterinarian for any illness or injury. I agree to hire a professional trainer approved by RGR Rescue and provide foundation training for the adopted dog including socialization. PROOF MUST BE PROVIDED within 14 days of the signing of this contract. The dog should begin training no later than 16 weeks of age. The adopted dog will be treated as a family member with loving care and affection. I will do my best to ensure the adopted dog’s safety and well-being. I hereby agree to care for the above described pet in a humane and responsible manner and to provide it with clean and adequate shelter, food, water, and all necessary and required veterinary care. RGR will provide the dog with the first 2 sets of vaccinations and initial deworming. Everything else, even during the foster period, will be the responsibility of the adopter including the 3rd set of vaccinations, spay/neuter, microchip, flea/tick medications, etc. The adopted dog will live inside my home and will not be isolated from the family. I will adequately walk the adopted dog on a leash or exercise the adopted dog in a fenced yard. I will never let the adopted dog run loose or roam, I will never keep the adopted dog chained or tied up, I will never keep it continuously in a yard, garage, patio, balcony, or pen, or leave the adopted dog outdoors, even in a fenced yard when no one is at home. I will not have the adopted dog’s ears cropped nor will I have its tail docked unless medically necessary. I will never allow the adopted dog to suffer any physical, mental, or emotional abuse. I/we affirm that no member of my household has been convicted of an animal welfare law violation such as neglect, cruelty, abandonment etc. I will ensure proper licensing of the adopted dog after 16 weeks of age and will attach a personal identification tag to the collar to be worn by the adopted dog. I will provide proof of this tag to RGR within 7 days of signing this contract. I will ensure compliance with all applicable local and state statutes. Proof of licensing will be provided to RGR between 16 and 18 weeks of age. I agree to transfer the adopted dog’s microchip into my possession with accurate contact information within 7 days of the end of the foster period. I will provide proof of this to. I agree to always keep information on the adopted dog’s microchip up to date and accurate. I will not use an e-collar or prong collar unless I have consulted with a representative from RGR or professional trainer and have been properly trained on how to use these tools, either with the adopted dog or with a previous dog. I am adopting the dog for myself and I agree to not give away, sell, or trade the adopted dog, even as a gift to a friend or family member. I will neither take the adopted dog to a shelter nor abandon the adopted dog. I understand that I must notify RGR, without delay, if I can no longer care for or keep the adopted dog and agree to give RGR, reasonable time to rehome the adopted dog or place the adopted dog in an approved foster home, if available. Reasonable time means no less than 14 days for adopted dogs located in California and 60 days for adopted dogs: adopt@realgood.dog located outside of California. I understand I will be responsible for any transport fees required to get the adopted dog back to the rescue. I agree to hire a professional trainer approved by RGR if any bite incident occurs. I agree to continue crate training the dog. I agree to never take the dog to an off-leash dog park, as I understand the risks associated with off leash dog parks. In the event the adopted dog becomes lost or dies, I will immediately notify RGR. I will also immediately notify RGR of any change of contact information (address, phone number, or email address). I agree to accept responsibility and ownership of the dog at my own risk, even during the foster period, and I release RGR and its agents or members from any and all liability arising out of possession and/or ownership of the dog. I agree that I am assuming total financial responsibility for the dog as of the date of this contract. RGR and its agents or members will not be held responsible for any damages or expenses (veterinary or other) incurred during my ownership of the dog. The adopted dog’s known background and medical history have been discussed with me. I understand that RGR has made no representation concerning the health, condition, training, behavior, or temperament of the dog. I hereby understand and agree that the rescue makes no representations or warranties, expressed or implied, about the above-mentioned animal's temperament, breed, or full-grown size and is hereby absolved from any liability for future damages or injuries caused by said animal. I also understand and agree that the rescue further gives no guarantees, expressed or implied, of the suitability of the animal to the adopter and/or his family. I AGREE to stay in communication with RGR regarding the adopted dog weekly during the foster period. After the dog is 16 weeks of age, I agree to stay in communication with RGR by sending updated pictures to once a month for the first year, and every 4 months every year after. I agree to permit RGR to make inquiry about and enforce any of the above conditions and requirements at any time during the foster period or after the adoption. This can include visits to my home and contact with my veterinarian. I UNDERSTAND THAT FAILURE TO COMPLY WITH ANY OF THE ABOVE PROVISIONS WILL RESULT IN FORFEITURE OF THE DOG TO Real Good Rescue I hereby agree that the adoption donation of $ 600 helps to cover the rescue’s expense of raising, caring, and vetting for/of the animal and is not refundable, except within the foster period time frame. Foster period ends once the dog is spayed/neutered. I agree to take the animal on a foster to adopt basis. I also agree to return the animal promptly if I decide not to adopt. I also understand if I fail to return the animal promptly, RGR reserves the right to take back the animal immediately. adopt@realgood.dog I restate my understanding of my agreement to allow a RGR representative to visit my home to do a home visit before and or after I adopt the animal. I restate my understanding that accidental animal bites or other injuries to humans and other animals do occur, and agrees to hold harmless and indemnify, and protect RGR from any claim or suit filed by anyone as a result of such an incident. In addition, RGR will not be responsible if animal should damage or destroy property belonging to me or others or shall transfer any disease or internal or external parasites to other animals belonging to me or other animals. I certify that all statements made by me on this adoption agreement are true and correct. I agree that the rescue has the right to confiscate the above described animal in the event that any statements made by me are found to be false or if I break the contract. I understand that by voluntarily signing this agreement, I am entering into a legal and binding contract with Real Good Rescue. Breach of any term(s) of this agreement is deemed actionable by Real Good Rescue. I understand the above conditions and agree to abide by them. I understand and agree that if I break this contract in any way or if a Real Good Rescue representative decides that I am no longer qualified to keep the animal, then the animal may be taken away from me by Real Good Rescue and I may be held accountable for any violation of this agreement.
